Carer escalation plans (in development)
Carers will have an escalation plan – that they keep themselves – to help them plan for urgent need in relation to predictable changes in the condition(s) of their cared-for person.
The escalation plan will be developed with the cared-for person and the services s/he uses.
It will be a record of what to do in the event of likely developments in the condition(s) of the cared-for person – for example the development of additional needs in progressive illnesses, falls in the case of a frail person, or a mental health crisis.
During 2018/19 we will be working with commissioners and statutory Health and Social Care agencies to design and pilot the escalation plan so that it is available widely from April 2019.
