Hospital planning
At some point during your time as a carer, either you or the person you care for may have an admission to hospital, whether it is planned or an emergency. Our hospital planning resources offers practical help, support, advice, problem-solving and signposting to ensure you and the person you look after can cope during the first few weeks at home following a hospital stay. If you find that you are struggling with your caring role, it may be beneficial to have a carers assessment.
Our booklets can give you advice, signposting and practical tips to help you before, during or after a stay in hospital, this could include:
- Advice to help you navigate to specialist support
- Knowing what to pack and what not to pack
- Benefits claim changes
- Hospital information
- Discharge planning & knowing your rights
- Practical support
The content is full of useful information given to us by carers and other health professionals. In addition, our helpline team is available to provide help with service navigation at this difficult time.
You can download and view the booklets below. There’s two versions of each booklet, one that is better viewed on a computer with clickable links and the other which is better for downloading and printing at home.
